Endogenous estrogens are largely responsible for the development and maintenance of the female reproductive system and secondary sexual characteristics.
Although circulating estrogens exist in a dynamic equilibrium of metabolic interconversions, estradiol is the principal intracellular human estrogen and is substantially more potent than its metabolites, estrone and estriol, at the receptor level.
Exogenous estrogens are metabolized in the same manner as endogenous estrogens.
Estradiol, estrone, and estriol are excreted in the urine along with glucuronide and sulfate conjugates.

WARNING: ENDOMETRIAL CANCER, CARDIOVASCULAR DISORDERS, BREAST CANCER and PROBABLE DEMENTIA Estrogen-Alone Therapy Endometrial Cancer There is an increased risk of endometrial cancer in a woman with a uterus who uses unopposed estrogens.
- PREMARIN vaginal cream is contraindicated in women with any of the following conditions:.
- Undiagnosed abnormal genital bleeding [see Warnings and Precautions (5.3) ].
- Breast cancer or a history of breast cancer [see Warnings and Precautions (5.3) ].
- Estrogen-dependent neoplasia [see Warnings and Precautions (5.3) ].
The most common adverse reactions ≥ 1% in the double blind phase are shown below (Table 1) [see Clinical Studies (14.1) ].
